Hello Shirley, 

I believe it is a W8 BEN form. I remember sending it to a few of clients while I was working in Accounts Receivable department of an MNC. That was back in year 2018 and unfortunately I cannot recall any specifics about the form except for the name.

We usually file the W-8 ECI for our foreign offices since their results are included in our federal income tax filings.  Much of this depends on who are you filing for, the foreign office or the U.S. parent and also what type of entity.
